Request a document
Overview
You can use this form to request documents from accounts that have been closed less than seven years ago that are unavailable online. To request a document, you must be the account owner or primary or joint cardholder.
If you need multiple documents, please submit a separate request for each document.

Home Loans
Closure letter
A letter confirming the home loan has been paid in full and closed, including the date of closure.
Statement(s) for closed home or investment loan
A copy of the final statement prior to closure, the last 12 months of statements before loan closure or statements issued in the last financial year before loan closure.
Personal Loans
Closure letter
A letter confirming the personal loan has been paid in full and closed, including the date of closure. For Secured loans this will include details of the security that was held.
Statement(s) for closed personal loan
A copy of the final statement, the last 12 months of statements before loan closure, or statements issued in the last financial year before loan closure.
Credit Cards
Closure letter
A letter confirming a credit card has been closed in accordance with the terms and conditions, including the date of closure.
Statement(s) for closed credit card
A copy of the last statement issued, or the last six or 12 months of issued statements.
Savings & Transactions Account
Closure letter
A letter confirming the account has been closed, including the date of closure (excluding Term Deposits).
What can’t you request?
Using this form, you won’t be able to request:
- Statements for closed accounts that are available through the Statements section in the Bank of Melbourne App or Internet Banking
- Closed credit card statements more than 12 months before the last issued statement
- Loan documents or agreements not included in the document list relating to your personal, investment or home loan – please complete and return the form Request for Copies of Documents/Contracts (PDF 58KB)
- Documents for a customer who is deceased – Dealing with bereavement
- Financial Assistance – Financial hardship
Accessing your document
Once your document is ready, we’ll email you with instructions for accessing it. You’ll have seven days to download the document from Share files in the Bank of Melbourne App or Internet Banking, after which time you’ll need to submit another request.
